Barney Frank, 86, said from hospice care that Democrats can win again only if they shed the image of backing politically unacceptable social change.
The former Massachusetts congressman argued left-wing overreach on trans rights, Medicare and candidate choices has alienated moderates and hurt the party.
Frank said many mainstream Democrats privately agree but stay quiet for fear of primary challenges and accusations that they are covert conservatives.
Despite congestive heart failure and what he called his "unfortunate situation," Frank said he remains hopeful about the party's future and expands on that case in a book due in September.
