Washington State Patrol said the collision near milepost 129 just north of SR 512 blocked all southbound lanes for two hours and 41 minutes.
The suspect was taken to Madigan Army Medical Center, while a 48-year-old woman and a 14-year-old passenger were hospitalised at Tacoma General and Mary Bridge.
Investigators said the northbound vehicle was travelling in southbound lanes before hitting another car near 84th Street; all three injured people were wearing seatbelts and further charges are pending.
