The Cincinnati Reds closer was hurt after one pitch in the ninth inning of a 3-2, 10-inning loss to the Chicago Cubs at Wrigley Field.
Manager Terry Francona said Pagán, unable to put weight on his leg, had warmed up normally; Jose Franco replaced him and escaped the inning before Chicago won in extras.
Pagán had felt the same hamstring on the final pitch of a win over San Francisco last month. The 34-year-old had 32 saves in 2025 but has a 6.43 ERA this season.
