The three-alarm blaze broke out around 12:30 a.m. on the second floor of a six-storey walk-up on Dyckman Street in Inwood, drawing more than 190 fire and EMS personnel.
FDNY said five people were taken to hospital in severe condition, while one firefighter suffered minor injuries; residents described alarms, screams and firefighters breaking windows as crews searched apartments.
Officials said open apartment doors worsened the spread, while closed doors limited damage. The Red Cross used an MTA bus for temporary shelter, and the fire's cause remains under investigation.
