The Centers for Medicare and Medicaid Services said the exposure lasted several weeks and began when providers or their representatives entered sensitive data in the wrong fields.
The publicly accessible database fed a directory meant to help seniors find doctors and medical providers that accept particular insurance plans, linking SSNs to names and other identifying information.
CMS said it is working to fix the problem. The portal was part of the Trump administration's DOGE technology modernisation push, launched with early guidance from Elon Musk.
