In an X post on Tuesday, the Massachusetts Democrat said thousands of civilians in Iran had died after Pentagon harm-reduction programmes were gutted.
Warren called Hegseth dangerous, urged Congress to rein him in and demanded an end to the war; the defence secretary did not immediately respond.
The criticism came as lawmakers also questioned the administration's Iran strategy after Iranian strikes reportedly caused $5bn damage to US bases across the Gulf and diplomacy over Hormuz remained stalled.
