More than three-quarters of 39 commercially used sweeteners altered the growth of at least one of 25 gut bacterial species in Cambridge laboratory tests, challenging the idea that low-calorie sweeteners are biologically inert.
The sharpest effect came from isosteviol combined with duloxetine, which strongly suppressed Roseburia intestinalis and Parabacteroides merdae—two bacteria linked to digestive, metabolic and immune health.
Researchers also found more than 100 cases where a sweetener's impact changed when paired with caffeine, flavorings, another sweetener or eight common drugs; 34 combinations intensified the effect and 68 weakened it.
In a synthetic 25-species microbial community, the isosteviol-duloxetine mix reduced overall microbial diversity and shifted species balance, with follow-up cell tests suggesting higher toxicity and altered inflammatory responses.
The team said the work was done in vitro, not in people, so human studies are still needed to test whether these microbiome changes occur at real-world doses and affect health.
