Summary
Shaun Burton, 60, suffered traumatic brain stem and chest injuries when his East Midlands Railway train hit the rear of another service near Elstow on 19 June, an inquest opening heard.
162 people were injured in the crash, including 102 taken to hospital, and Burton was pronounced dead at the scene at 18:49 BST before being formally identified on 25 June.
RAIB's interim findings said Burton's train passed a red signal moments before the collision, with braking applied for about nine seconds as speed fell from roughly 76mph to 49mph at impact.
Investigators also found the train ahead had stopped unexpectedly because of a fault with its Automatic Warning System equipment.
The inquest in Ampthill was adjourned pending full reports from the Rail Accident Investigation Branch and British Transport Police.
