Q: With federal sanitation oversight weakened, who now guarantees a cruise ship is safe for its next passengers?

The main federal backstop is still the CDC’s Vessel Sanitation Program, not the cruise line alone. Although the program was disrupted by layoffs in 2025, the cuts were later reversed and a core team of U.S. Public Health Service officers kept inspections going. The VSP continues to conduct unannounced inspections of cruise ships calling at U.S. ports, tracks outbreaks, and can require corrective action before a ship sails again. Ships are generally inspected twice a year and must score at least 86 out of 100 to pass. By 2025, the CDC had actually increased inspections to 273 ships, up sharply from 2024, and it has tightened some health rules, including around water testing. That means no single entity “guarantees” safety in an absolute sense, especially with a fast-spreading virus like norovirus. In practice, safety for the next passengers depends on layered oversight: the CDC’s sanitation inspections and outbreak monitoring, the cruise line’s own cleaning, isolation and reporting protocols, and additional checks from flag states, port authorities and classification bodies. In the Ruby Princess case, Princess Cruises said it isolated sick passengers and crew, intensified sanitation, consulted the CDC and disinfected the ship before its next departure. Whether that is sufficient is ultimately judged through this broader oversight system, with the CDC’s Vessel Sanitation Program remaining the key U.S. public-health authority.

Princess Cruises said the outbreak hit during a 20-day voyage, prompting enhanced sanitation measures across the ship.
