Dengue infection usually gives strong long-term protection only against the same serotype, not the other three. So people exposed in 2023-2025 to DENV-3 may still be vulnerable to DENV-2. That matters because a second dengue infection with a different serotype can be more severe than the first. In these cases, older antibodies may recognize the new virus but fail to neutralize it. Instead, they can help the virus enter immune cells more efficiently, a process known as antibody-dependent enhancement. This can raise viral levels in the body and trigger a stronger inflammatory response. The result is a higher risk of severe dengue, including plasma leakage, bleeding, shock, and organ complications. Health experts are especially concerned because DENV-2 has long been associated in many outbreaks with more severe disease, particularly in secondary infections. In Sri Lanka’s case, the strain shift means the country is not facing only more mosquito exposure after cyclone-related flooding and persistent rain. It is also facing a population-level immunity mismatch: many people have some dengue immunity, but to the wrong serotype. That combination helps explain why the return of DENV-2 is considered especially dangerous.

More than 52,000 dengue infections and 31 deaths were recorded in Sri Lanka by June 27, marking a sharp nationwide outbreak this year.
DENV-2 now makes up about 75% of confirmed cases, and health officials said immunity built during years when DENV-3 dominated has left many people more exposed.
