Review of 5 Trials Finds GLP-1 Drugs May Boost Fertility in Obese Men

Five randomized trials reviewed at ENDO 2026 found GLP-1 drugs did not harm testosterone, sexual function or sperm quality in men 18 to 65, easing a key safety concern for obese men using Ozempic-like medicines.
Two studies suggested benefits beyond safety: a 24-week semaglutide trial improved sperm morphology, while a 16-week liraglutide trial raised testosterone and related hormones in obese men with weight-linked low testosterone.
The findings challenge routine testosterone replacement for obesity-related hypogonadism, because TRT can sharply cut sperm counts within 3 to 6 months and sometimes stop sperm production altogether.
Researchers said GLP-1 drugs may work by reducing fat-driven hormonal suppression and possibly through direct receptors in testicular tissue, but the evidence remains limited, heterogeneous and unapproved for infertility treatment.
Independent experts said the results apply only to high-BMI men, warned rapid weight loss can temporarily suppress fertility, and called for larger human studies before prescribing practice changes.
