Q: Why must men with breast cancer rely on treatments designed for women?

Men with breast cancer usually receive treatments developed for women because the disease is extremely rare in men, leaving too few patients for large, male-specific clinical trials. Male breast cancer accounts for less than 1% of all breast cancer cases, so most of the evidence on what works comes from studies in women. Doctors can still use those treatments because the cancers are often biologically similar. In men, the most common form is invasive ductal carcinoma, the same common type seen in women, and most male cases are hormone receptor-positive, meaning they respond to endocrine therapies already widely used in female breast cancer. Staging and many treatment principles are also the same. That is why men are commonly treated with the same core options: surgery, chemotherapy, radiation, hormone therapy and targeted drugs. In practice, physicians adapt these established protocols to male patients rather than relying on a large body of male-only research that does not yet exist. The problem, specialists say, is not that these treatments are irrelevant to men, but that the evidence base is thinner. Because men are underrepresented in trials, doctors have less data on the best drug combinations, dosing strategies and long-term outcomes specifically for male patients. That is why awareness, earlier diagnosis and greater inclusion of men in breast cancer research are seen as critical to improving care.
