More than 223,000 UK Biobank participants were analyzed, and people whose blood metabolite profile appeared older than their actual age faced higher dementia risk—especially if they also carried the APOE risk gene.
Nearly 4,000 participants developed dementia during follow-up, and the strongest association was with vascular dementia, with the MileAge delta marker also tied to earlier-onset and unspecified dementia.
Researchers said the blood-based "MileAge" measure may capture modifiable biological aging, pointing to cardiovascular risk control, physical activity and mental health as ways to potentially lower risk.
The study was observational, used a single blood measurement and drew from a healthier, mostly European-ancestry cohort, so it shows association rather than causation and the biomarker still needs clinical validation.
