Max Fried will undergo tests in New York on Thursday after leaving the Yankees’ 7-0 loss to Baltimore with posterior soreness in his left elbow after 61 pitches.
The 32-year-old said the pain felt like hyperextension in the back of the elbow, and manager Aaron Boone said he was "not that concerned" after speaking with him.
An orthopedic surgeon interviewed by The Athletic said posterior elbow pain is not uncommon for pitchers, especially those with prior Tommy John surgery, and could point to triceps tendinitis, bone spurs or loose bodies.
Doctors are also expected to check Fried’s UCL because posterior pain can be an early warning sign of ligament trouble, though the surgeon said an intact ligament with increased signal would be a more typical finding.
Any extended absence would hit a Yankees rotation that expects Gerrit Cole back soon; Fried entered Wednesday with a 2.91 ERA in nine starts, but lower velocity and shakier command than in 2025.
